The problem
Insurance claims moved between hospital, patient and insurer through email, phone calls and physical paperwork. Nobody could answer where a specific claim had reached without asking someone who might be off shift, and the documentation attached to a claim varied depending on who had assembled it.
The cost showed up twice. Staff spent hours a day on status-chasing calls that produced no clinical value, and claims were rejected for missing documents that had been requested but never tracked.
